Dr. Kamell, Dr. Bodnar and our staff would like to welcome and thank you for choosing our office for your dermatology care. We hope that the following information below helps to answer some frequently asked questions regarding our practice.

New Patient Registration
We ask our new patients to arrive 15 minutes before your scheduled appointment to register with our office and allow us time to verify insurance benefits on your behalf.
Prescription Refills
Please contact our office between 7:30 AM and 11:30 AM at for refills. We do not refill prescriptions when the office is closed as access to your medical records is not available outside of office hours.
Emergencies
During the week, call . Weekends, we ask you to contact your local emergency room or urgent care for emergency medical care.
Medical Services
Our office limits its practice to Dermatology. This includes all diseases and conditions that affect the skin and subcutaneous tissue. This includes Rashes, Acne, the Scalp, Nail Problems, Moles, Warts, Cysts, Lipomas, and Skin Cancers. We also perform Mohs Micrographic Surgery for certain types of skin cancer.
The initial visit can include consultation, lab tests, diagnostic services or minor surgical procedures. We do reschedule some surgical appointments to a later date when additional time is needed to allow for the proper management of our patients' medical care.
Laboratory Services
Complete bloodwork, fungal cultures and microscopic pathology exams are usually performed and billed through our office unless our patients specifically request a lab preference.
Cosmetic Services
- Mild to Medium-depth Peels - Used for improvement in skin texture, vary in price depending on type and depth of peel.
- BOTOX® Injections - These are injections of a medication into the frown lines around the eyes and forehead (crows feet) used to relax the muscles of these areas for 3-4 months.
- Restylane®, Perlane®, Juvéderm™ Injection - Filling substances that improve deep facial lines.
- Sclerotherapy - Injections of solutions for leg veins to eliminate them.
- Vasculight Laser and Intense Pulse Light - For the treatment of blood vessels of the face, irregular mottling of the skin and the removal of pigmented spots.
- Smoothbeam Laser - For the treatment of fine lines around eyes without "down time", and for the treatment of certain types of Acne.
- Dermabrasion and Laser Resurfacing - Used for the aggressive treatment of facial lines.
- Facials/Microdermabrasion - For conservative low risk rejuvenation.
Payment for Services/ Insurance Billing
Payment is expected at the time services are rendered unless prior arrangements have been made. If we are a member of your insurance plan, we will submit a claim on your behalf with payment due according to the appropriate provider contract. A current insurance card must be presented prior to services being rendered if you wish to have your insurance billed. Any office visit or surgical copay is due at time of service.
Please be aware that the filing of insurance claims does not guarantee payment by your carrier. Frequently, proof of eligibility, covered and non covered services and various exclusions cannot be determined until your carrier processes the claim. Therefore, we do submit insurance claims with the understanding that you are responsible for any non-covered services as well as co-payments and deductibles.
Medical Records Release Policy
We are pleased to comply with all requests to release copies of your medical records. A clerical fee of $6.00 per quarter hour will be charged for making the record available, as well as a $0.25 fee per page for the copying of the record. Additional fees may be incurred due to postal expenses.
We do reserve the right to prepare your records for release and have you contact an outside copy service for records exceeding 50 pages.
